IRS TAX RETURN TRANSCRIPT REQUEST PROCESS 2015-2016
If you have received a letter from the college indicating that you have been selected for verification and you are required to provide a federal tax return transcript, please use the instructions in this PDF document to request that information from the IRS. Tax filers can request a transcript, free of charge, of their “2014” tax return. Please be sure to request a “IRS Tax Return Transcript” and not the IRS Tax Account Transcript.

SATISFACTORY ACADEMIC PROGRESS APPEAL FORM
If you wish to appeal your current Satisfactory Academic Progress Status please complete the Satisfactory Progress Appeal Form and submit it to the Financial Aid Office.
2015/2016 BOOK ADVANCE PROGRAM INSTRUCTIONS
Financial aid recipients who have financial aid funds remaining after the payment of tuition and fees may use the excess to purchase (or rent) books and supplies from the WMCC bookstore during the book advance period, which generally starts 10 days prior to the beginning of the semester. The 15/16 Fall Semester Book Advance Program begins Friday August 21, 2015 and concludes on Friday, September 25th, 2015.
- Students must complete the WMCC Title IV Authorization form to participate in the electronic book advance process. This form must be turned into the Financial Aid Office and be completed annually. If turning in the Authorization Form after the start of the Book Advance process, please allow at least (1) business day for processing.\
The Financial Aid Office must have received your signed Award Notification Letter.
The Business Office must have received your Deferred Payment Agreement form.
You must be due a refund.
If you accepted the loan portion of Financial Aid you need to go online to www.studentloans.gov and complete the Loan Entrance Counseling and the Master Promissory Note (MPN).
Federal Work Study Forms and Information
W-4 EMPLOYEE'S WITHHOLDING ALLOWANCE CERTIFICATE FORM
This must be completed prior to beginning work study employment. Complete this form so that the correct federal income tax can be withheld.
I-9 EMPLOYMENT ELIGIBILITY VERIFICATION FORM
This must be completed prior to beginning work study employment. Additional documentation must be provided to establish identity and employment authorization.
FEDERAL WORK STUDY PROGRAM PAYROLL AUTHORIZATION FORM
This must be completed prior to beginning work study employment. This form must be signed by student and supervisor.
WORK STUDY BI-WEEKLY TIMESHEET
This form is to be completed by the work study student or supervisor.
CONSORTIUM AGREEMENT FORM
If you are registering for courses at another college within our system and want your financial aid applied to the tuition and fee charges, this form needs to be completed and returned to the WMCC Financial Aid Office.
