SPRING 2012 BOOK VOUCHER FORM AND INSTRUCTIONS
Use this form if you have (1) completed and returned your Award Notification Letter to the Financial Aid Office, (2) have completed and returned your Deferred Payment Agreement Form to the Business Office, and (3) you are expecting a refund and want to be able to purchase your books based on your expected refund. Federal Work Study Forms and Information

W-4 EMPLOYEE'S WITHHOLDING ALLOWANCE CERTIFICATE FORM (PDF 100 KB)
This must be completed prior to beginning work study employment. Complete this form so that the correct federal income tax can be withheld.

I-9 EMPLOYMENT ELIGIBILITY VERIFICATION FORM (PDF 380 KB)
This must be completed prior to beginning work study employment. Additional documentation must be provided to establish identity and employment authorization.

FEDERAL WORK STUDY PROGRAM PAYROLL AUTHORIZATION FORM (PDF 36 KB)
This must be completed prior to beginning work study employment. This form must be signed by student and supervisor.

WORK STUDY BI-WEEKLY TIMESHEET (PDF 36 KB)
This form is to be completed by the work study student or supervisor.

Direct Loan Processing Guide and Information

If you have signed your Award Notification Letter accepting a Direct Loan (subsidized or unsubsidized or both), please read the information contained within this form carefully. Your financial aid cannot be processed until you have taken these very important steps.

Consortium Agreement

CONSORTIUM AGREEMENT FORM (PDF 132 KB)
If you are registering for courses at another college within our system and want your financial aid applied to the tuition and fee charges, this form needs to be completed and returned to the WMCC Financial Aid Office.
